Southwest Modern Medicine's Direct Primary Care is a membership model of healthcare. For one low monthly fee, you can have unlimited access to your primary care provider for routine and most urgent care needs.  Please see our pricing model below: 

I want to thank you for being a part of our practice and for trusting me with your care. Since opening in January 2024, I have not increased membership rates, even as the cost of medical supplies, technology, and overhead has risen. I have worked hard to absorb these increases while continuing to provide extended visits, same- or next-day access, direct messaging, and a level of personalized care that is simply not possible in traditional insurance-based models.

To continue offering this level of service and to maintain the financial health of the practice, SWMM will be updating membership rates effective January 1, 2026 to:

• $65/mo. for ages 5–29
• $80/mo. for ages 30–64
• $95/mo. for ages 65+

If you are a current member, your membership will remain at the current rate for the first three months of 2026.

Your adjusted rate will begin April 1, 2026.
